This form must be filled out in its entirety. Once the option for your child has been PAID, player's names will be manually added to the AA team's tryout/evaluation page, but these names will not appear publically. Your child's name will be indicated with a number, which you will receive at evaluations (not prep skates). If and when a player has been released from AA, their name will automatically be transferred to the next category in that division. You will be provided with a different tryout number at the next evaluations. THERE ARE TWO PAYMENT OPTIONS-SportsPay or etransfer. Once you submit this form you will be taken to a SportsPay payment page where you have the option to pay with either Visa or MasterCard. The second payment option offered is via an etransfer. If you choose that option, payment must be sent to
